Most of the problems with veins arise when valves which control the direction of blood flow fail. Two basic reasons for vein valve failure are dilation of the veins so that the valve leaflets can no longer close or the valve leaflets deteriorate. When either of these situations occurs, blood tends to fall back down which causes excessive pressure in the veins below. As more valves fail, more pressure develops and this leads to more vein failure.

Typically, vein failure occurs in the superficial vein system. This leads to the formation of visible and often uncomfortable veins in the legs. Occasionally, veins in the pelvis will fail which causes bulging veins in the groin area including the genitalia and the upper thighs. Deep vein failure usually occurs as a consequence of a deep vein thrombosis (blood clot) and, rarely, is the result of a congenital disorder.
